PATIENT HISTORY
Mr. S, a 52-year-old man with a distinguished career, appears at your private practice office for an appointment dressed very well, polished and fashionable. He relates a history of very severe recurrent depression for the past 20 to 30 years, with many individual depressive episodes lasting from 3 to 6 months. At times, he says, he became so desperate that he put one of the guns from his gun collection in his mouth. However, he experienced periods of high functioning without depression between these episodes.
